Now Accepting Applications for Service Dogs

Thanks to The Allegheny County District Attorney’s office, the Autism Center of Pittsburgh is once again accepting applications for a Service dog!  Service dogs are very special animals that enjoy roles as both a companion and helper. They serve their partners and are loyal and affectionate.  A service dog will help your child live more independently in your home and in public.

We give dogs away twice a year, Spring and Fall. We are currently taking applications at this time, all applications are kept on file. Please download the application below and mail it to us.

 

Eligibility Requirements:

You must be a resident of Allegheny County to be awarded the dog.

 

To Apply:

1. Print and complete the Application for Service Dog.

2. Include the following documents with your application:

A recent photo

Your latest tax return

3 letters of reference from someone other than a family member

A professional letter of reference from a doctor, therapist or psychologist

Child’s latest psychological evaluation

Any supporting evidence pertaining to wandering or elopement issues in general


Return the application and all supporting documents to:

About Dr. John Carosso

Dr. Carosso’s experience in the mental health field dates back to over twenty-five years in a number of capacities including as a Licensed Clinical Psychologist and School Psychologist. He has worked in private, school-based, inpatient, residential, and in-home settings. His specialization is in the evaluation, assessment, and treatment of children and teenagers and, in that regard, he has conducted literally tens of thousands of evaluations with children, and families, of all walks of life.
